My dog is pee pad train and she starting to pee in the carpets and in the bedroom, what should I do?

Hello, I have a 1 year old chihuahua dog, who is a inside dog. since she was a pup we trained her to go use a pee pad. she was doing real good and been doing real good. lately she been acting up real bad, she started peeing on the carpet in the living room, we have a guest room and she pooping in there too. Two days ago she peed on the bed twice. she usually sleeps with us, but then i woke up to pee on the bed, seems like she too lazy to go to her pee pad, when i look at her pee pad she pees there every once in a while, but to me she seems like she getting lazy, yesterday we were watching a movie in the living room and she pee right in front of us in the carpet like nothing and ran cause she knows she is in trouble , when her pee pad is 3 feet from her, brand new pee pad. I really don't know what else to do, i put sprays to lure her to her pee pad, like i use when she a pup, i added more pee pads. we have cats in the house, so she good with them, she the only dog. just extra information just to know her daily activities are, she hates to go outside or be tied up, we let her outside and she is right by the door crying. very timid dog. I am king in my last strings with this dog, her name is martini. we were even wanting to give her to someone, but i love this dog so much. so please if you have any advise please comment. thank you
